The clinical significance of ANCA in Kawasaki disease

摘要 目的探讨抗中性粒细胞抗体(ANCA)在川崎病(KD)中的临床意义。方法对广州市儿童医院2005-11-01—2006-11-01收治的98例KD患儿血清进行ANCA检测,分析KD中ANCA的阳性率,ANCA与静脉注射丙种球蛋白(IVIG)敏感度、冠状动脉损害程度的关系。结果ANCA在KD患儿中阳性率为69.4%;KD中ANCA阳性组和ANCA阴性组两组间对IVIG的敏感性差异无统计学意义(P>0.05);而ANCA阳性患儿冠状动脉损害程度重于ANCA阴性患儿(P<0.05),差异有统计学意义。结论ANCA在KD发生冠状动脉损害的病理发生发展中起一定的作用,可作为临床中KD发生冠状动脉损害的一项预警的实验室指标。 Objective To probe into the clinical significance of anti-ncutrophil cytoplasmic antibody (ANCA) in children with Kawasaki disease (KD). Methods Serum ANCA was measured in 98 children with a diagnosis of Kawasaki disease from November 1,2005 to November 1,2006. We evaluated the relationship between results and IVIG responsiveness,and coronary abnormalities in Kawasaki disease. Results The positive ANCA in KD was about 69, 4%. Statistical analysis showed that the positive ANCA was not related to the responding of IVIG ( P 〉 0. 05 ) , but was related to the development of coronary abnormalities( P 〈 0. 05 ). Conclusion ANCA perhaps plays an important role in the development of KD in children. ANCA contributes to the development of coronary abnormalities.
