摘要
目的介绍压疮及高危人群压疮评估/报告表的设计及应用。方法根据Braden量表评分标准设计高危人群压疮评估/报告表,包括高危人群压疮发生评估表,压疮发生危险因素量化评估表,难免压疮申报表,压疮评估表,压疮预防及治疗措施五部分并应用于临床。结果设计的表格经过临床应用,评价科学,使用方便,取得良好的效果。结论高危人群压疮评估/报告表有利于压疮的规范管理,提高了护理人员控制压疮发生的预防意识,降低压疮发生率,缩短带入性压疮的愈合时间,促进了护理工作的持续改进。
Objective To introduce the design and application of bedsore evaluation and report form for high risk population. Methods Based on the scoring criteria on Braden Scale, we designed bedsore evaluation and report form for high risk population, quantified scale for bedsore risk factors, bedsore report form and prevention and treatment of bedsore. Result The designed evaluation forms were found to be convenient and simple for clinical practice by way of clinical application. Conclusions The bedsore evaluation and report form for high risk population is suitable for normative management of bedsore. By using it, nurses can improve their awareness of prevention and control of bedsore so that the incidence of bedsore can be lowered and the nursing care can be promoted continually.
