摘要
目的:探讨伸肌腱自发性断裂的原因及治疗的新方法。方法:通过14例患者手术中局部所见,分析肌腱断裂与局部解剖结构的关系,以及通过趾短伸肌游离移植重建伸肌腱后患指功能恢复状况。断裂指伸肌腱均行趾短伸肌游离移植重建,移植肌腱的远端编织缝合,近侧端端缝合,以趾短伸肌肌腹包埋吻合口。结果:14例患者均进入结果分析,随访2～5年,①外形:14例重建手指外形基本正常,其中,有1例出现轻度桡偏、1例出现轻度屈曲,无过伸畸形。②使用情况:在工作和生活中,14例均能自然地使用患指。③力量:14例中手指背伸力量基本恢复,有6例术后3个月内自觉患指背伸力量减弱,但并未影响患指的远期功能恢复。14例指间关节、掌指关节关节外形满意,关节稳定,畸形消失,手指活动灵活。结论:通过探讨伸肌腱自发性断裂的原因、特点,选择趾短伸肌游离移植治疗伸肌腱自发性断裂,结合术后中药熏洗、功能锻炼,能够获得满意治疗效果,同时也为肌腱移植提供了一种有效方法。
Objective：To approach the reason and new therapy of extensor tendon spontaneous disrupt.Methods：Through 14 patients＂ intraoperative part findings,we would analyze the relation between tendon rupture and opography structure.14 patients＂ extensor tendon were all repaired with museulus extensor digitorum brevis tendon free grafting. Museulotendinous distal end grafted was sew up like braid,and the proximum was sew up by end to end.The anastomosis stoma was embed by the muscle belly of museulus extensor digitorum brevis.Results：After 2-5 years follow-up, artieulationes interphalangeae straighten is normal, and same with Uninjured side. The ruction of broken ends after tendon transplantation repairing was well.Conclusion：h is a effective method to repair extensor tendon spontaneous rupture with museulus extensor digitorum brevis free grafting.
