耳蜗负和电位与动作电位比值与膜迷路积水 被引量:3

Ratio of Negative Summating Potential to Action Potential in Cochlea and Hydrolabyrinth

摘要 为探讨耳蜗负和电位与动作电位(-SP/AP)比值变化与膜迷路积水的关系,测试44例Meniere病(病侧59耳,健侧29耳)和52例非Meniere病(半面痉挛24例,三叉神经痛28例;病侧53耳,健侧51耳)的耳蜗电图。结果Meniere病组病侧耳的-SP/AP比值与同组健侧耳、非Meniere病组病侧耳及健侧耳的-SP/AP比值相比都明显升高(P<0.001)。22例Meniere病还做了甘油试验,结果纯音听力甘油试验阳性率为56.3%,耳蜗电图甘油试验阳性率为81.3%,服甘油后比服甘油前-SP/AP比值显著下降(P<0.001),表明-SP/AP比值变化与膜迷路积水程度有密切关系。行耳蜗电图检查及甘油试验,有助于Meniere病的诊断和确立治疗方案。并对耳蜗电图检查及甘油试验的机理及少数变异结果进行讨论。 To explore the ratio of negative summating potential (-SP) to action potential (AP) in cochlea and hydrolabyrinth,electrocochleogram(ECochG)was detected in 44 patients with Meniere disease (59 diseased and 29 healthy ears)and 52 non-Meniere disease patients (including 24 cases with semi-facial spasm and 28 cases with trifacial neuralgia,53 diseased and 51 healthy ears). The results showed that the -SP/AP value on patient's diseased ears of MD was significantly higher than that of patient's healthy ears of MD and patient's diseased and healthy ears of non-Meniere disease(P<0. 001). Glycerol Test(GT)was performed in 22 patients with MD. According to selfestablished standard, positive rates of PTA and -SP/AP value in diseased ears was 56. 3% and 81. 3% .respectively. -SP/AP value of diseased ears remarkably decreased after taking glycerol(P<0. 001) ,but in healthy ears, there was no significant difference in -SP/AP value before and after taking glycerol(P>0. 05). The results suggested that there was a close relationship between changes of -SP/AP value and degree of hydrolabyrinth. E-CochG and GT were helpful for the diagnosis and the selection of treatment measures of MD. The different results of GT were analyzed and discussed in detail.
