点时间尿蛋白浓度与24小时尿蛋白总量相关性比较 被引量:5

Correlation Between Protein in Spot Urine Sample and Quantitation of 24-hour Proteinuria

摘要 【目的】研究用点时间尿蛋白浓度(UPC)替代24 h尿蛋白总量(UP)监测及评估尿蛋白排泄情况的可行性。【方法】311例慢性肾脏疾病(CKD)患者分别按性别、年龄和尿量分组,收集其点时间(7Am,7Pm)和24 h尿液,测定其中UPC并计算UP;测定相应时间点尿肌酐(Cr),计算尿蛋白/尿肌酐值(P/CR)。探讨点时间尿液UPC、UP、P/CR与24 h尿液相应指标的相关性。【结果】①点时间尿UPC、P/CR与24 h尿UPC、P/CR呈正相关,P均<0.001,7Am相关性最好;②点时间UPC与24 h UP的相关性高于点时间P/CR与24 h UP的相关性;③Cr<10 mL/min时,点时间UPC、P/CR与24 h UP无相关性。【结论】点时间7Am UPC和P/CR测定可代替24 h UP对CKD患者的尿蛋白进行检测及评估,其中点时间尿蛋白与24 h尿蛋白定量的相关性高于尿P/CR的相关性,此测定适用于不同性别、年龄、尿量的人群,但Cr<10 mL/min时不推荐使用。 [Objective] To investigate the feasibility of replacing 24-hour urine protein(UP) monitoring by measuring urine protein concentration (UPC) in spot urine specimen for evaluating urine protein excretion. [Methods] Spot urine samples at special time points (7Am & 7Pm) and 24-hour urine samples were obtained from.311 patients with chronic kidney disease(CKD) for detecting UPC and UP. Urine creatinine in corresponding time points was measured and protein-to-creatinine ratio(P/CR) was calculated. All patients were divided according to different gender, age and the amount of urine. The correlation between UPC, UP,P/CR and urine corresponding parameters was analyzed. [Results]OThere was positive correlation between UPC, P/CR in spot urine samples and those in 24-hour urine samples ( P 〈0. 001), and there was strong correlation in 7Am.②The correlation of UPC in spot urine samples to UP in 24-hour urine sample was stronger than that of P/CR in spot urine time to UP in 24-hour urine sample. (~)There was no correlation between UPC, P/CR in spot urine samples and those in 24-hour urine samples when Ccr〈10 mL/min. [Conclusion] Detection of UPC and P/CR in 7Am spot urine sample can replace UP in 24-hour urine sample to evaluate urine protein of CKD patients, and the correlation of UPC in spot urine sample to 24-hour UP is stronger than that of P/CR. Urine protein detection in spot urine sample can be applied in population with different gender, age and amount of urine, but can not be used when Ccr(10 mL/min.
