高血压患者肾动脉阻力指数与临床指标的相关分析 被引量:1

摘要 目的了解原发性高血压患者肾动脉阻力指数(ResistiveIndex,RI)的变化,分析肾动脉RI与临床指标的关系。方法选择原发性高血压患者168例。对照组51例。用彩色多普勒超声仪测量肾主动脉、肾段动脉和叶间动脉的RI,比较其差异;测量彩超检查当天的收缩压(SBP)和舒张压(DBP),计算脉压(PP);检测血清尿素氮(BUN)、血清肌酐(Scr)及内生肌酐清除率(Ccr);分析肾动脉RI与临床指标的相关性。结果高血压组患者各级肾动脉RI均明显高于对照组(P=0.002~0.004)。相关分析显示,肾动脉RI与BUN、Scr呈正相关,与Ccr呈负相关;肾动脉RI与PP呈正相关、与年龄呈正相关;多元逐步回归表明,各级肾动脉RI增高均与年龄呈正相关、与Ccr呈负相关关系。结论原发性高血压患者的肾动脉RI与年龄和Ccr相关,而且肾动脉RI还可以间接反映全身动脉硬化的状况。 Objective To investigate the changes of renal arterial resistive index (RI) in patients with hypertension, To analyze the correlation between renal arterial RI and clinical characteristics. Methods 168 cases of primary hypertensive patients were selected. 51 subjects also were studied as control group. Renal arterial RI were detected in all cases by Color Doppler Flow Imaging. Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were measured on the same day of the examination. Pulse pressure (PP) was calculated. Blood urea nitrogen (BUN), serum ereatinine (Scr) and ereatinine eleartmee (Cer) were measured in all eases. Results Renal arterial RI of various renal arteries in hypertension group increased more significantly than that in control group (P=0.002- 0.004). The correlations showed that renal arterial RI were positively correlated with BUN and Scr. Renal arterial RI were negatively correlated with Ccr. Renal arterial RI were positively correlated with pulse pressure and age. The multiple regression showed renal arterial RI were positively correlated with age and negatively correlated with Ccr. Conclusions Renal arterial RI were correlated with age and Ccr in patients with hypertension. Renal arterial RI can reflect the state of arteriosclerosis in the body.
