摘要
目的探讨小儿结核性脑膜炎的CT表现及其对临床预后的评估价值。方法对32例证实的小儿结核性脑膜炎,回顾性分析其临床病程、不同时期的CT和预后,观察CT征象、病变范围变化。结果CT表现主要为脑积水、脑萎缩、脑膜结核瘤、血管周围异常强化、脑梗塞等,后遗症期包括脑积水、局限脑软化灶、脑膜钙化等。其中脑积水是小儿结核性脑膜炎最早的异常表现,其程度与病程持续时间成正比,与年龄大小成反比。结论小儿结核性脑膜炎的CT征象具有特征性,在诊断中具有重要价值,结合临床可对预后作出初步评价。
Objective To explore the CT manifestation and evaluate its value in clinical prognosis of infant tuberculous meningitis. Methods Thirty - two cases were diagnosed as tuberculous meningitis clinically, and the change of CT manifestation in distinct periods was retrospectively analyzed, compared with their clinical duration and prognosis. Results CT manifestation of tuberculous meningitis in children included hydrocephalus, encephalanalosis, tuberculoma of meninges, perivascular enhancemen and cerbral infarction. Sequela included hydrocephalus ,focal area of atrophy secondary to infarcts and meningeal calcification. Hydrocephalus was the first abnormal and its degree was directly related to the duration of the illness while inversely related to patients＇ age. Conclusions The CT manifestation of infant tuberculous meningitis is significant and has important value in the diagnosis, and it can make preliminary assessment on prognosis associated with clinical signs.
