摘要
目的探讨乳化异氟烷预处理对心肌缺血再灌注损伤的保护作用。方法雄性新西兰兔32只,体重2.0—2.5kg,随机分成4组(n=8):心肌缺血再灌注组(对照组,C组)、脂肪乳组(L组)、异氟烷组(I组)和乳化异氟烷组(Ⅱ组)。每组均缺血1h,再灌注3h。实验结束后测定血清磷酸肌酸激酶(CK)和乳酸脱氢酶(LDH)活性,电镜观察心肌组织超微结构的变化。所有数据以均数±标准差(x±s)表示,采用单因素方差分析,并用Dtmnett检验进行两两比较,差异有统计学意义(P〈0.05)。结果与C组比较,Ⅰ组和EI组血清CK和LDH降(P〈0.05),而L组无明显变化。电镜下c组和L组线粒体水肿,破坏明显,而Ⅰ组和EI组线粒体损伤较轻。结论乳化异氟烷预先给药体对兔心肌缺血再灌注损伤有明显的保护作用。
Objective To investigate effects of 8% emulsified isoflurane on ischemia-reperfusion injury of cardiac muscle in rabbits. Method Thirty-two male adult New Zealand white rabbits, weighing 2.0 to 2.5 kg, were randomly divided into four groups (eight for each group) : ischemia-reperfusion group (C group), Intralipid group (L group), isoflurane group (I group) and emulsified isoflurane group (El group). Every rabbit was subjected to 1 hour of ischemia followed by 3 hour reperfusion. Blood and myocardial specimens were obtained at the end of the reperfusion for determination of lactate dehydrogenase (LDH), lactate dehydrogenase (CK) activity and electron microscopic examination. All data were expressed as mean ± standard deviation of mean (SD). Data analysis between groups was performed using multiple analysis of variance (ANOVA) for repeated measures with post hoc analysis by the Dtmnett test for multiple pairwise comparisons. Differences were considered significant when P was 〈 0.05. Results The activation of LDH and CK were significantly declined in the I group and El group as compared with their corresponding control group ( P 〈 0.05). There was no significantly different between the control and L group. The histopathological changes of mitochondria and ultrastructure of myocardium produced by myocardial ischemia-reperfusion injury were also significantly attenuated by isoflurane and emulsified isoflurane. Conclusion 8 % emulsified isoflurane is an anesthetic preconditioning agent, which could protect the myocardium against the ischemia-reperfusion injury.
