摘要
报告了对6具新鲜尸体的枕颈部骨韧带标本通过截骨术复制成齿状突Ⅱ型骨折的模型,采用生物力学方法测量了寰枢关节的三维运动范围。结果表明,齿状突骨折对寰枢关节的影响主要表现为后伸及侧屈运动范围增大,分别由正常的21.50°和7.25°增至45.42°和20.81°,增加了52.7%和65.1%,与正常相比均有显著性差异(P<0.01)。实验结果提示,齿状突骨折导致的寰枢关节不稳主要表现为后伸和侧屈运动,治疗应以恢复齿状突的完整性和增加寰枢关节的稳定性为重点。
Six fresh cadaver specimens of occipital-atlanto-axial complex were reproduced type I odontoid process fracture and the three-dimensional movenent of C1-C2 were measured by biomechanics method. The result indicated that there were increased rang of motion(ROM)in extension and lateral bending at C1-C2 after odontoid fracture. The average of the extension and lateral bending ROM were 21. 50?and 7. 25?in normal.and 45. 42?and 20. 81?in odontoid fracture .increased 52. 7% and 65.1% respectively. The ROM of C1-C2 had greatly significant differences between normal and odontoid fracture in extension and lateral bending (P< 0. 01). The result suggested that the instability of C1-C2 after odontoid fracture were mainly in extension and lateral bending. For treatment.it is very importment to restore the intact of odontoid process or to increase the stability of C1-C2.
