脑钠肽对左室舒张功能不全诊断价值的研究 被引量:18

摘要 目的:探讨脑钠肽(BNP)在单纯左室舒张功能不全患者中的变化规律,为早期诊断左室舒张功能不全提供依据。方法:60例单纯左室舒张功能不全的患者,依据舒张功能障碍的程度依次分为3个亚组:松弛功能减低组(20例),假性正常组(21例),限制型充盈障碍组(19例)。测定其血浆脑钠素-32浓度并与20例健康查体者(对照组)进行比较。结果:左室舒张功能不全组血浆BNP浓度高于对照组(P<0.01)。而且3个亚组血浆BNP浓度均较对照组高(P<0.05)。血浆BNP的浓度随着左室舒张功能障碍程度增加而升高(P<0.05)。有心衰症状组较无心衰症状组血浆BNP浓度高(P<0.01)。BNP用于舒张性心衰诊断的受试者工作特征(ROC)曲线下面积为0.843(95%CI,0.764~0.922,P<0.05)。结论:单纯左室舒张功能不全患者的血浆BNP浓度升高,且与疾病程度有关。BNP可作为左室舒张功能不全的诊断标准之一,其升高程度可提示疾病的严重性。 Objective: To study the change of plasma brain natriuretic peptide (BNP) in patients with left ventricular diastolic dysfunction (LVDD),and establish the method for early diagnosis of LVDD by plasma BNP. Methods: Sixty patients with LVDD were classified as impaired relaxation (n = 20), pseudonormal (n = 21 ) and restrictivelike filling (n = 19) groups. The levels of plasma BNP were measured in patient groups and compared with those of 20 healthy subjects (control group). Results: Compared with control group, the levels of plasma BNP were significantly elevated in impaired relaxation, pseudonormal and restrictivelike filling groups (P 〈 0.05). Within 3 subgroup, BNP levels were increased with the severity of the LVDD (P 〈 0.05). Patients with LVDD and overt symptoms had higher BNP levels than those with asymptomatic LVDD (P 〈 0.01 ). The area under the receiver-operating characteristic curve for BNP to detect any diastolic dysfunction was 0.843 (95%CI,0.764-0.922 P 〈 0.05). Conclusion: The level of plasma BNP was increased in patients with LVDD and normal systolic function. The level of BNP was related to the severity of the LVDD. The level of BNP can be regarded as one of early diagnostic criteria of LVDD.
