大剂量静脉丙种球蛋白治疗葡萄球菌烫伤样皮肤综合征疗效探讨 被引量:16

摘要 目的:探讨静脉注射丙种球蛋白对葡萄球菌烫伤样皮肤综合征的疗效。方法:回顾性分析2003年3月至2007年5月收治的葡萄球菌烫伤样皮肤综合征共75例,根据治疗方法不同分为两组,对照组35例给予抗生素治疗,治疗组40例在对照组基础上加用静脉丙种球蛋白1g/(kg·d),连用1—2d,对两组患儿体温正常时间、皮肤红斑及疱疹停止发展时间、皮肤疱疹干燥时间、住院天数进行分析比较。结果:治疗组体温正常时间、皮肤红斑及疱疹停止发展时同、皮肤疱疹干燥时间、住院天数分别为(2.9±1.46)d、(6.03±1.44)d、(3.87±1.37)d、(3.08±1.14)d,对照组分别为(4.86±2.67)d、(8.43±2.92)d、(6.09±2.33)d、(4.51±1.9)d,治疗组显效率及总有效率分别为80.0%和100%,对照组显效率及总有效率分别为37.1%和71.4%,两组比较差异均有统计学意义(P〈0.01)。结论:大剂量丙种球蛋白静脉注射治疗葡萄球菌烫伤样皮肤综合征疗效显著,可快速改善症状及体征,并提高治疗效果。 Objective: To study the effect of intravenous immunoglobulin (IVIG) in the treatment of staphylococcal scalded skin syndrome (SSSS). Methods: From March 2003 to May 2007, 75 patients with SSSS were hospitalized in the children' s hospital of Chongqing medical university. As the control group, 35 cases had used antibiotic, and other 40 cases were treated with antibiotic and MG as treatment group,with IVIG 1 g/(kg· d) for 1 -2 days. Then the time of normal body temperature, of the skin erythema and herpes stopping growing and to dry, and their duration in hospital were comparied. Results: The time of normal body temperature, of the skin erythema and herpes stopping growing and to dry, and their duration in hospital were ( 2.9 ± 1.46 ) days, (6.03 ± 1.44) days, ( 3.87 ± 1.37 ) days, ( 3.08 ± 1.14) days in the treatment group, while (4.86 ± 2.67 ) days, (8.43 ± 2.92) days, (6.09 ± 2.33) days, (4.51± 1.9) days in the control group. The total effective rate and the rate of excellent results in the treatment group were 100% and 80.0%, but 71.4% and 37.1% in the control group. There was a significant difference between the two groups ( P 〈0.01 ). Conclusious: High IVIG is effective in the treatment of SSSS,which can rapidly improve the chief symptoms and promote the curaitive rate.
