摘要
目的 探讨实时灰阶超声造影诊断慢性下肢静脉功能不全的可行性及临床价值。方法对26例30条有慢性下肢静脉功能不全临床表现的患者分别进行实时灰阶超声造影和下肢静脉顺行造影，对照二者诊断深静脉和穿支静脉反流以及穿支静脉定位的一致性，分析超声造影诊断慢性下肢静脉功能不全的准确性和优缺点。结果与下肢静脉顺行造影结果相比，超声造影判断深静脉瓣膜逆流的符合率为96．6％，交通静脉符合率为93．3％。超声造影可以精确定位交通静脉并可显示静脉瓣膜逆流情况。结论超声造影可观察下肢静脉全貌，方便、准确、安全，可用于诊断下肢慢性静脉功能不全。
Objective To investigate the feasibility and clinical significance of diagnosis on chronic venous insufficiency in lower limbs by contrast-enhanced ultrasound. Methods Twenty-six patients with thirty sides of lower limbs were examined by real scaled contrast-enhanced ultrasound and anterograde venography. The consistency between the reflux of deep veins and perforating branch veins and the orientation of perforating veins performing by the two methods were compared. The accuracy, advantages and disadvantages of contrast-enhanced ultrasound in the diagnosis of chronic venous insufficiency of lower limbs were analyzed. Results The accuracy of contrast-enhanced ultrasound in examining deep venous reflux and perforating veins were 96.6% and 93.3 % respectively, compared with the result of anterograde venography. Contrast-enhanced ultrasound could precisely orient the perforating veins and show venous contraflow. Conclusions Contrast-enhanced ultrasound can observe a full view on veins in lower limbs. It is a convenient, accurate and safe method, and can be used to diagnose chronic lower limbs venous insufficiency.
