急性缺血性脑卒中患者超早期基质金属蛋白酶表达与临床预后的关系 被引量:14

The correlation between the ultra-early matrix metalloproteinases expression levels and the prognosis of acute ischemic stroke patients

摘要 目的研究急性缺血性脑卒中超早期基质金属蛋白酶(MMPs)表达水平同患者临床预后的关系。方法选择发病6h内完成血标本收集的急性颈内动脉系统脑梗死患者47例,分为临床结局良好组(24例)和临床结局不良组(23例)。MMPs指标包括MMP-2、MMP-9和MMP-13;90天后随访改良的Ranking量表(mRS)评分,临床结局良好定义为mRS 0-1分,临床结局不良定义为mRS 2-6分。结果临床结局良好组患者MMP-2、MMP-9和MMP-13的表达水平分别为(241.88±69.34)μg/L、(80.83±67.24)μg/L和(74.25±26.51)ng/L,临床结局不良组患者分别为(243.57士88.48)μg/L、(147.39±132.64)μg/L和(94.80±61.49)ng/L,仅MMP9在两组间的差异有统计学意义(P=0.039)。结论急性缺血性脑卒中超早期MMP-9表达水平有助于判断患者的临床预后,而MMP-2和MMP-13水平同患者临床预后无关。 Objective To evaluate the correlation between the ultra-early matrix metalloproteinases (MMPs) expression levels and the prognosis of acute ischemic stroke patients. Methods A total of 47 acute ischemic stroke patients whose blood samples collected within 6 hours after onset were studied. The patients were divided into good clinical outcome group (24 patients) and bad clinical outcome group (23 patients). MMP-2,MMP-9 and MMP-13 were detected. All patients were followed up for three months after onset and modified Ranking scale (mRS) scores were recorded. Good clinical outcome were defined as mRS 0--1 and bad clinical outcome were defined as mRS 2-6. Results The average expression levels of MMP-2, MMP-9 and MMP-13 in good clinical outcome group were (241.88±69.34)μ/L, (80.83±67.24)μg/L and (74.25±26.51) ng/L re- spectively,while they were (243.57±88.48)μg/L,(147.39±132.64)μg/L and (94.80±61.49)ng/L respectively in bad clinical outcome group. Only MMP-9 expression was significantly differ- ent(P=0.039) between the two groups. Conclusion Ultra-early MMP-9 expression level may help to judge the prognosis of acute ischemic stroke patients, while there is no correlation between early MMP-2 and MMP-13 expression levels and prognosis.
