摘要 目的采用TCD评估大脑中动脉(MCA)区脑梗死后的血流动力学变化。方法对35例经头颅磁共振证实的MCA区急性脑梗死患者(病例组)和30例健康对照组行TCD检查。结果病例组患侧MCA流速低于健侧,两侧MCA流速比值低于对照组,两组间有显著性差异;病例组患侧与健侧大脑前动脉(ACA)流速比值高于对照组,两组间有显著性差异;病例组双侧大脑后动脉(PCA)流速比值跟对照组相比无显著性差异;所有病例组患侧MCA脉动指数减低(P<0.001)。结论TCD提供的血流动力学参数可以部分反映MCA病变后的侧支循环情况,MCA区脑梗死后侧支循环代偿以ACA为主。 Objective To evaluate hemodynamics changes in patients with cerebral infarction of middle cerebral artery (MCA)territory.Method Thirty five patients with acute cerebral infarction of MCA territory confirmed by MRI were selected to serve as patient group and thirty healthy persons were selected as controls. All subjects were measured by TCD.Results In patient group,the blood flow velocity of MCA in lesion side was lower than the other, the blood velocity ratios of MCA was lower than ratios in controls,there was a significant difference between the two groups;The ratios of anterior cerebral artery for lesion side to the other in patient group was higher than ratios in controls,there was a significant difference between the two groups; There was no significant difference for the ratios of posterior cerebral artery between the two groups; All pulsatility index of MCA in lesion side in patient group was decreased ( P〈0.001).Conclusion TCD findings can partly reflect the collateral patterns in patients with MCA stenosis or occlusion.ACA was the primary collateral circulation in patients with cerebral infarc- tion of middle cerebral artery territory.
