摘要
目的应用站立海绵垫的方法干扰人体直立时的本体觉,探讨后半规管良性阵发性位置性眩晕(be-nign paroxysmal positional vertigo of the posterior semicircular canal,BPPV-PSC)患者在本体觉受到干扰时的姿势平衡。方法39名确诊为BPPV-PSC患者进行以下四种感觉条件下的姿势稳定性定量测量:T1:睁眼站立于坚硬平板;T2:闭眼站立于坚硬平板;T3:睁眼站立于海绵垫;T4:闭眼站立于海绵垫,所有测试均于姿势描记仪上完成。采用身体直立时足底压力中心的平均晃动速度(sway velocity,SV)为研究参数,并与39名正常人姿势稳定性作为对照。结果①应用海绵垫干扰本体觉后,BPPV-PSC组睁眼(t=11.905,P<0.001)及闭眼(t=11.457,P<0.001)时的SV均增加,对照组睁眼(t=9.701,P<0.001)及闭眼(t=17.604,P<0.001)时的SV亦均增加;②相同感觉条件下两组SV比较,除T1外(t=1.810,P=0.078),T2~T4条件下BPPV-PSC组的SV均高于对照组,差异有显著统计学意义(t=3.279~5.177,P=0.002~0.000)。结论BPPV-PSC患者的姿势稳定性在视觉和/或本体觉受到干扰时低于正常人,其机制可能与患者半规管流体动力学变化导致的前庭终器传入感觉改变有关。
Objective To investigate the postural instability in the patients with benign paroxysmal positional vertigo of posterior semicircular canal (BPPV--PSC) when the somatosensory input was interrupted by standing on foam surface. Methods 39 patients with BPPV--PSC were taken the posturography including four conditions, such as firm surface with eyes open (T1); firm surface with eyes closed (T2); foam surface with eyes open (T3); foam surface with eyes closed (T4). The body sway velocity (SV) recorded by the posturography was analyzed as parameter of postural stability. The postural stability of the patients with BPPV- PSC was compared with that of the matched control group. Results After interrupted the somatosensory input by the standing foam surface, the SV of both BPPV--PSC and control groups decreased significantly (P〈0. 001). The SVs between BPPV--PSC and control group were significantly different for the test condition T2 to T4 (t=3. 279-5. 177, P=0. 002-0. 000), except in the T1 (t=l. 810, P=0. 078). Conclusion The decreased postural stability was presented in the patients with BPPV--PSC when visual or somatosensory input for upright standing was interrupted, which may due to change of input from the vestibular end organ because of modification of canal dynamics in the posterior semicircular canal in patients with BPPV--PSC.
