摘要
对80例慢性呼吸衰竭病人和37例健康人的红细胞(RBC)膜带3蛋白结构、含量和阴离子转移功能、RBC内外血气和血中HCO_3^-、Cl^-进行了研究。结果提示:肺心病Ⅱ型呼吸衰竭病人带3蛋白阴离子转运蛋白的氨基酸组分无变化,但带3蛋白阴离子转运功能降低,HCO_3^-/Cl^-交换受限,红细胞内产生的大量HCO_3^-不能充分排出,使红细胞内pH(pHi)升高。RBC内相对偏碱对缓解高碳酸血症所引起的氧离曲线过度右移,具有重要意义。
The changes of structure and content of the erythrocyte membrane band 3 protein and its function of anion transport and blood gases inside and out of the erythrocyte were observed in 3 groups. Group Ⅰ type Ⅰ, respiratory failure (n=40); group Ⅱ, type Ⅱ respiratory failure (n=40) and control group (CG, n = 37). The results showed that structures of anion transport site of erythrocyte membrane band 3 protein might not change in the patients of cor pulmonale with type Ⅱ respiratory failure, but their anion transport function markedly decreased and HCO_3^-/Cl^- exchange was restricted. This prevented HCO_3^- from moving out of RBC and led to relative alkolosis in the red cells. This might benefit the oxygen dissociation curve which was moved extremely to the right by hypercapnia.
