摘要
目的对导致脑深部电刺激(DBS)手术不成功的原因进行分析,探讨防治措施。方法随访并回顾性的分析本中心自1998年至2003年间为49例患者进行的DBS手术及术后资料。结果术后出血1例,术后电极连接点处皮肤溃烂1例,手术未完成1例,术后开机无效2例,术后效果显著低于术前评估4例,术后1年内效果减弱3例。结论进行多学科协作的术前患者筛选,药物测试,正规严谨的手术操作,定期的随访程控,合理的术后用药指导可以很大程度的提高DBS手术的成功率。
Objective To evaluate and analyze the reasons for deep brain stimulation （DBS） failure in patients of Parkinson＇ s disease （PD）, and to make recommendations to improve DBS outcomes. Methods Forty-nine patients who had undergone implantation of DBS devices in our institution from 1998 to 2003 were evaluated, and the complete medical record was reviewed. Results Twelve （24. 5% ） patients suffered from unsuccessful DBS. One patient hemorrhaged, one patient ulcerated at the joint of electrode and wire, one patient had uncompleted surgery, two patients had no access to programming, four patients had poor access to programming, three patients had decreased access during one year postoperative. Conclusions We summarized the important points that manage patients with DBS more successfully, which include rigorous of triage, screening of candidate, consummate surgery, reasonable postoperative follow-up and guidance of medication.
