胰炎合剂对大鼠重症急性胰腺炎肠屏障影响的实验研究 被引量:2

Effect of Chinese herb Yiyanheji on intestinal mucosa barrier in rats with severe acute pancreatitis

摘要 目的研究胰炎合剂对重症急性胰腺炎(SAP)大鼠肠黏膜屏障和肠道细菌移位的影响。方法选择健康Wistar大鼠,雌雄不限,体质量(250±30)g,共90只,随机分为假手术组(SO,n=30)、模型组(SAP,n=30)、胰炎合剂治疗组(SAP+YH,n=30)。通过向大鼠胰胆管内逆行注射5%牛磺胆酸钠(0.1ml/100g)制备成SAP模型,术后分别用0.9%氯化钠注射液、胰炎合剂进行灌胃治疗。术后72h处耍大鼠,剪取肠系膜淋巴结,称重后分别匀浆,做细菌培养.观察匀浆体外培养后的细菌量、细菌移位率;取回肠末段标本,剪成1mm×3mm小条块,用4%戊二醛固定,用作透射电镜检查,观察末端回肠组织电镜下变化。应用统计学软件对数据进行分析。结果假手术组有15%的肝脏细菌培养阳性,胰炎合剂组细菌移位率为22.5%,明显低于SAP组的90.0%(P〈0.01);胰炎合剂组的细菌移位量为(1.98±0.97)×10^3CFU/g,明显低于SAP组(P〈0.01);电镜下观察SAP组回肠黏膜微绒毛稀疏脱落,排列紊乱,细胞内部分线粒体肿胀,细胞间紧密连接增宽。胰炎合剂组回肠黏膜微绒毛排列整齐.略变短稀疏,细胞内线粒体肿胀,细胞间紧密连接增宽不明显。结论胰炎合剂有明显减轻SAP时肠黏膜损害,保护肠黏膜屏障,抑制肠道细菌移位的作用。 Objective To study the effects of Chinese herbs Yiyanheji on intestinal mucosa barrier in rats with SAP. Methods Animal models of SAP were induced by retrograde injection of 5 % sodium taurocholate (0. 1ml/100g) into the common biliopancreatic duct. 90 healthy Wistar rats weighing (250 ± 30)g were randomly divided into 3 groups: sham operation group (SO, n = 30 ), SAP group (SAP, n = 30 ) and Chinese herbs Yiyanheji treated group(SAP + YH, n = 30). Treated group was treated with Yiyanheji after operation. The SAP group was treated with physiological saline. Three groups of rats were killed at 72 hours after operation or treatment. Bacterial cultures were performed in all animals. The changes of terminal ileum' tissue were observed by optical electron microscopy. The data of test were analyzed by statistic software. Results The incidence of bacterial translocation was 22.5 % in Yiyanheji treated group,and it was lower than that in SAP group which was 90.0%. The difference was significant(P〈 0.01). The quantity of bacteria in liver issue in Yiyanheji group was (1.98 ± 0.97) × 10^3CFU/g. That was lower than that of SAP group( P 〈 0.01 ). In rats with SAP, The intestinal epithelial microvilli were rare, exfoliative, and deranged. The tight junctions were damaged. Yiyanheji could obviously improve the pathological changes. There were no obvious dilation in the central ehyle ducts. The damagement of tight junctions between interstitial cells was gentle. Conclusion Chinese herbs "Yiyanheji" had effects on protecting intestinal mucosa barrier and preventing bacterial translocation from intestinal following severe acute pancreatitis.
