摘要
目的提高脂肪含量极少肾血管平滑肌脂肪瘤的术前诊断水平。方法对9例脂肪含量极少肾血管平滑肌脂肪瘤患者的临床资料进行分析并复习相关文献。结果9例患者CT平扫示高密度衰减,增强后肿块密度均质增加。3例行MR IT1加权像未示高信号,在T2加权像示均质的低信号。B超检查6例示等回声、3例低回声。术前5例诊为肾血管平滑肌脂肪瘤,4例误诊为肾细胞癌。术后所有患者病理诊断为脂肪含量极少肾血管平滑肌脂肪瘤。结论并非所有肾血管平滑肌脂肪瘤能检测到脂肪成分,CT、MR I、B超相结合能提高脂肪含量极少肾血管平滑肌脂肪瘤术前诊断的正确性,但病理诊断是最终依据。
Objective To elevate the preoperative diagnosis level of renal angiomyolipoma （RAML） with minimal fat. Methods Clinical data of 9 eases of RAML with minimal fat were retrospectively analyzed and the relative literature was reviewed. Results A high degree of attenuation on unenhanced CT imaging and enhancement on contrast-enhanced CT images were observed in all nine patients. Hypointensity on T2-weighted MR images were observed in 3 cases. Isoechogenicity and hypoechogenicity were observed in six of nine and three of nine patients, respectively. By color Doppler examination, the tumors showed abundant pulsatile blood vessels in three patients. Four patients with renal angiomyolipoma with minimal fat were misdiagnosed as renal cell carcinoma. All patients were pathologically confirmed as RAML with minimal fat. Conclusion The detection of fat in a renal lesion is highly suggestive of angiomyolipoma, but fat is not detected in all angiomyolipoma. CT combined with MRI and sonograms can elevate preoperative diagnosis accuracy of RAML with minimal fat, however, this disease should be confirmed by pathologic diagnosis.
