摘要
目的探讨双水平气道正压通气(BiPAP)联用纳洛酮治疗老年慢性阻塞性肺疾病(COPD)并Ⅱ型呼衰的疗效。方法对我院2005年1月～2007年8月收治的老年COPD并Ⅱ型呼衰患者共132例,按入院顺序单双号分为对照组和试验组,在常规治疗基础上,对照组用BiPAP,试验组BiPAP联用纳洛酮1.2克加入0.9%氯化钠溶液100ml中静滴Q12h共3天,观察两组治疗前后患者心率、呼吸、血SaO2、PaCO2、PaO2、PH值、神志等的变化。结果治疗2h后两组患者的所有观察指标都有明显改善(P<0.01),48h内试验组与对照组比较,改善更加明显(P<0.01、P<0.05)。对照组总有效率为84.8%,试验组总有效率为95.5%。结论BiPAP联用纳洛酮治疗老年COPD并Ⅱ型呼衰有较好疗效。
Objective To study the clinical effect of BiPAP and Naloutong on treating Chronic Obstructive pulmonary Disease （COPD） complicated with Ⅱ Respiratory failure in aged patients. Method One hundred and thlrty-two aged patients with COPD complicated with Ⅱ Respiratory failure who were in our hospital during January,2005 to August,2007. according to being hospitalized ordinal number, They were divided into that odd number was experimental group and double was control group. Two group were treated on the basis of convention, The control group based on that was treated with BiPAP and experimental group was treated reappear with naloutong injection iv Q12h in one course of treatment of three days. observing HR、RR、PaCO2 ,PaO2 ,PH and expression . Results All items of experimental group were improved rapidly（ P 〈 0, 01 ） after ventilating 2hours, expression resuscitated was shorter than control group （P 〈 0.01 ,P 〈 0. 05 ）, in 48hours. effective rate of control group was 84, 8% and experimental group was 95.5%, Conclusion The clinical effect of BiPAP and naluotong on treating COPD complicated with Ⅱ Respiratory failure in aged persons have good effect,
