摘要
目的比较不同透明角膜切口位置对白内障超声乳化术后角膜散光及视功能的影响。方法白内障患者60例(60眼)随机分成两组,对照组行上方11点方位透明角膜切口,研究组按最大角膜屈光度子午线方位做角膜切口,分别行超声乳化摘除联合人工晶状体植入术,术后第1周、第1个月及第3个月回访患者,行医学验光及角膜地形图检查。结果术后第1周、第1个月及第3个月两组患者之间矫正球镜后最佳视力对比差异无显著性(P>0.05)。术后第1周对照组角膜散光度明显大于研究组角膜散光度(P<0.05),对照组术后第1个月角膜散光度明显大于术后第3个月角膜散光度(P<0.05),研究组术后第1周角膜散光度明显大于术前及术后第3个月(P<0.05),术后第1个月较术前、术后1周及术后第3个月差异无显著性(P>0.05)。结论根据角膜地形图的角膜散光轴向选择透明角膜切口对角膜散光影响小,早期可获得较好视功能。
Objective To compare different clear corneal incision positions in phacoemulsification and the effects on postoperative astigmatism. Methods Sixty eyes of 60 patients were selected to undergo phacoemulsification and IOL implantation. Patients were randomly divided into a control group （incision on the steepest corneal meridian） and a test group （incision on the superior position）. Results One week, 1 month and 3 months after the operation, vision corrected with spherical lenses was not significantly different when the two groups were compared （P〉 0.05）. One week after the operation, corneal astigmatism in the control group was significantly higher than in the test group （P 〈 0.05）. Corneal astigmatism in the control group one month postoperatively was significantly higher than at 3 months （P 〈 0.05）. There was no significant difference in the test group at 1 month and 3 months （P 〉 0.05）. Conclusion A clear corneal incision on the steepest corneal meridian is a useful way to achieve better visual quality and postoperative corneal astigmatism can be reduced significantly.
