摘要
目的对比腹膜透析（PD）与血液透析（HD）患者接受肾移植的效果。方法从1997年7月～2007年2月接受同种异体肾移植术患者中选取28批次总共125例，每批次中均同时含有术前接受PD和HD的患者。按肾移植术前的主要透析方式分为PD组（44例）与HD组（81例），并对两组的临床资料进行比较。结果移植前PD组血浆白蛋白（ALB）水平明显低于HD组[（38．91±5．13）g／L和（43．78±4．82）g／L，P〈0．05]，但移植后两组ALB水平差异无统计学意义。PD组移植后早期移植肾功能恢复优于HD组，两组的术后18个月血肌酐水平分别为（84．00±19．94）μmol／L和（107．17±52．47）μmol／L，差异有统计学意义（P〈0．05），但远期差异无统计学意义。两组在移植前后的血红蛋白、尿量、排斥发生率、移植术后并发症、人，肾存活情况方面差异均无统计学意义。结论PD患者肾移植术后早期移植肾功能恢复较HD患者满意，但两者肾移植远期效果相当，均可作为终末期肾病患者肾移植的术前准备。
Objective To compare the effects of peritoneal dialysis（PD） and hemodialysis（HD） on patients of renal aUograft survival outcome. Methods Twenty-eight batehs including 125 renal recipients who received kidney transplants from July 1997 to February 2007 were retrospectively analyzed. Each batch containing both PD patients and HD patients simultaneously. According to the predominant renal replacement therapy modality pretransplantation,the 125 recipients were divided into PD group （a = 44） and HD group （a = 81 ）, and the clinical data of the two groups was compared. Results The pre-transplantation albumin level in PD group was （38.91 ± 5.13） g/L and （43.78 ± 4.82） g/L in HD group （P 〈 0.05）, but no obvious difference was found after transplantation. The aUograft function was better in PD group than that in HD group during the first year post-transplantation, which was statistically obvious in the 18th month for the serum creatinine level was （84.00 ± 19.94） μ mol/L in PD group and （ 107.17 ± 52.47） μ mol/L in HD group, respectively （P 〈 0.05）. There were no significant differences in the hemoglobin levels,urine volumes in peritransplantion period,rejection rates, complications,long-term aUograft functions and aUograft/reeipient survival conditions. Conclusion PD has a better short-term aUograft function than HD,and the effects of renal transplantation between the two methods of renal replacement treatment are similar. Both PD and HD are feasible for preparation before renal transplantion.
