摘要
目的总结球囊辅助弹簧圈栓塞技术治疗脑动脉瘤的经验。方法60例患者均经全脑血管造影明确有宽颈脑动脉瘤,均使用球囊辅助弹簧圈栓塞技术进行治疗。结果本组共栓塞脑动脉瘤63枚,球囊和微导管到位成功率达100%。3例在放置弹簧圈时发生动脉瘤出血,其余动脉瘤栓塞均顺利(2例需联合支架治疗)。术毕造影动脉瘤达致密栓塞50枚,次全栓塞8枚,大部栓塞5枚。出院时,3例术中出血病人1例恢复良好无后遗症,1例留有肢体偏瘫,另1例因出血较重昏迷不醒而放弃治疗自动出院。其余病人中以SAH起病的有10例留有轻度的神经功能障碍,另47例病人恢复良好。1年以上DSA随访病人12例,均未见动脉瘤的明显复发。结论使用球囊辅助技术有利于宽颈动脉瘤的弹簧圈填塞,扩大了动脉瘤栓塞治疗的适应证,提高了栓塞的安全性和可靠性。
Objective To summarize the experience on the treatment of cerebral aneurysms with balloon-assisted coiling technique. Methods 60 patients who had been confirmed to have wide-necked aneurysms by cerebral angiography were all treated with balloon-assisted coiling technique. Results In this group, a total of 63 aneurysms were embolized with 100% success for the deployment of balloon catheters and microcatheters. 3 cases of aneurysms bled in the process of being coiled, while all other cases of aneurysms were successfully embolized （2 cases combined with stents ）. Post-procedure angiography revealed that 50 cases of aneurysm were totally obliterated, 8 sub-totally occluded and 5 incompletely occluded. When the 3 patients who bled during the procedure were discharged from the hospital, One of them made a good recovery with no sequelae, while another remained with hemiparalysis, and the third one gave up treatment due to coma. Among the remaining patients with the onset of SAH, 10 were left with slight neurological deficit, and the other 47 patients recovered very well. 12 patients were followed up with DSA after one year and no obvious aneurysm recurrence was found. Conclusion Balloon-assisted coiling technique is beneficial to the coil embolization of wide-necked aneurysms. It has expanded the indication range of aneurysm embolization treatment, and enhanced safety and confidence in the embolization.
