摘要
目的:介绍水力分离技术在白内障囊外摘除中应用,并评价其优越性。方法:对249例(261只眼)施行手术,以改良信封式截囊,在前囊下、皮质间、皮质与核间注水多层分离,水力娩核、双腔灌吸、人工晶体植入、撕囊。结果:术后1周视力≥0.5者175眼(68.6%),3月视力≥0.5者229眼(87.5%)。术后部分虹膜后粘连20只眼,人工晶体夹持2只眼,以上并发症发生在外伤性白内障与陈旧性虹膜炎瞳孔膜闭合并白内障术者。结论:水力分离技术简便、安全,可减少娩核阻力,避免囊袋撕裂。同时可减少后发障的发生。
Objective:To introduce a hydro dissection technique in extracapsular cataract extraction and to evaluate its advantage in clinic.Methods:261 eyes of 249 patients were operated on with removed envelope capsulatomy,and with hydro dissection under anterior capsular,intercortexs,between cortex and nucleus,following by hydro delivery nucleus,double tubes'pour,intraocular implantation,capsulorhexis.Results:Visual acuities ≥0.5 one week after operation were obtained in 175 eyes(68.6%),and 3 months after operation in 229(87.5%).Part posterior synechiae of iris ocurred postoperately in 20 eyes,and pupillary capture in 2.These complications ocurred in traumatic cataracts and cataract complicated with obsolete iridocyclitis.Conclusions:Hydro dissection technique may reduce nuclear delivery resistance,avoid capsular bag rupture and prevent after cataract.
