摘要
目的:研究胰头静脉弓的CT表现,获得有关血管直径的正常值,探讨相关临床意义。材料与方法:随机选取129例连续性无中上腹区域恶性肿瘤及炎性病变的CT增强扫描病例,观察胰头静脉弓各属支走行及CT表现,测定血管直径。结果:胰头静脉弓属支包括胰十二指肠上后及上前静脉、胃结肠干、结肠右上静脉和胃网膜右静脉等,CT显示率分别为63.7%、77.5%、69.8%、70.5%和34.9%;血管直径均值分别为2.28、2.31、4.85、3.01和3.88mm。结论:正常人中上腹CT增强扫描常可清楚显示胰头静脉弓属支。对此静脉弓的认识有助于胰腺癌的术前分期及正确评估门脉系统血液动力学改变。
Purpose: To study the normal CT appearances of the venous arcade of the pancreatic head (VAPH) and to establish the values of their normal sizes, and to address the related clinical implications. Materials and methods: 129 consecutive cases without neoplastic or inflammatory diseases of upper abdomen who underwent contrast abdominal CT scan were randomly included into the study. The courses of VAPH major tributaries——posterior and anterior superior pancreaticoduodenal veins (PSPDV, ASPDV), gastrocolic trunk (GCT), right superior colic vein (RSCV) and right gastroepiploic vein (RGEV) were observed and vessel sizes carefully measured. Several cases of hepatic or pancreatic malignancy with involvement of VAPH were also observed for comparison. Results: The CT visualization rates of PSPDV, ASPDV, GCT, RSCV and RGEV were 63.7%,77.5%,69.8%,70.5%and 34.9% respectively, the mean values of measured vessel diameters being 2.28, 2.31, 4.85, 3.01 and 3.88 mm in the same order. Anatomic variation of GCT was present in 11 cases. In the control patient group, tributaries of VAPH demonstrated abnormalities in sizes and / or courses. Conclusion: Most tributaries of VAPH can be readily visualized on abdominal CT scans of normal individuals. Correct recognition and assessment of VAPH may be useful in the preoperative staging of pancreatic tumors and in understanding the hemodynamic alterations of the extrahepatic portal venous circulation.
