血管密度对颊粘膜鳞癌转移的预测价值 被引量:4

VALUE OF VASCULAR DENSITY IN BUCCAL MUCOSAL SQUAMOUS CELL CARCINOMA PREDICTING METASTASIS

摘要 收集65例颊癌手术标本及临床资料,正常人颊部组织作对照,通过免疫组化方法定量研究了颊癌内血管密度与其区域淋巴结转移和远处转移的关系。结果发现:颊癌内血管密度明显高于正常颊部,且随着局部淋巴结转移和全身转移的出现,其血管密度也明显增加;血管密度达到30条/mm2时,是颊癌转移发生的警界值,以此值预测颊癌转移的发生,敏感性达89.19%、特异性达82.14%、准确率为86.84%,具有良好的阳性预测价值,为临床上选择合理的治疗方案,提供可靠的依据。另外。 There are close relationships between the metastasis and angiogenesis in solid malignant tumor.For the purpose to prove whether the condition of angiogenesis in buccal carcinoma could predict the lymphatic and/or distal metastasis.This experiment collects the clinial informations and specimens of 65 cases in contrast to buccal tissues of normal adult,and with histologcial immunohisto-chemical and statistical methods,relations of the vascular density in buccal carcinoma with cervical lymphatic and distal metastasis were studied quantitively,The results showed that the vascular density in buccal tissues of normal adult was 9.17±3.40/mm 2,while the vascular density in buccal carcinoma was 20.14±8.23/mm 2 and terribly higher than in normal buccal tissues and increased accompaning appearance of lymphatic and distal metastasis.Density of 30/mm 2 was the warning value of carcinomatous metastasis.This value offered a sensitivty of 89.19%,specificity of 82.14% and accuracy of 86.84% to predict carcinomatous metastasis and could provide a reliable basis of selecting rational clinical therapeutic plan.It includes three possible reasons about the mechanism of angiogennesis in buccal carcinoma related to lymphatic and vascular metastasis.New vessels were fragile in structure and were invaded easier than normal vessels by cancer cells; Functions of inhibitive oncogenes would become lower when angiogenesis in cancer got heavier.So,the malignant behaviors became strong and promoted cancer cells' metastasis;Endothelia at ends of new growing capillaries could release collageneses and plasminogen activators which promoted cancer cells to separate and invade in new vessels.In addition,angiogenesis in carcinoma could promote cancer cells to invade adjacent tissues and give cancer cells more chances to destroy the lymphatic vessels.Meanwhile,cancer cells could enter lymphatic vessels to take place metastasis directly and indirectly by vessels.
