摘要
目的评价手法小切口白内障手术(MSICS)人工晶状体植入联合小梁切除术(三联手术)治疗青光眼合并白内障的效果。方法57例(64眼)青光眼合并白内障施行此三联手术,巩膜隧道切口入前房,巩膜瓣呈倒梯形,术中使用劈核方法,术后观察视力、眼压、滤过泡及并发症情况。结果随访3～18月,乎均6.5月,矫正视力达0.3及以上者48眼(75.00%),6眼(9.38%)由于青光眼性视神经萎缩术后视力差。眼压控制在正常范围内者58眼(90.63%),54眼术后均形成了功能型滤过泡,在不用降眼压药物的情况下眼压正常,平均眼压由术前的(33.90±6.09)mmHg降至术后的(15.57±3.41)mmHg。术中术后未见严重并发症。结论三联手术具有切口小、组织损伤轻、能良好控制眼压、视力恢复快及并发症少等优点。一次手术即可达到增进视力,降低眼压的双重目的。
Objective To evaluate the therapeutic effect of the combined operation of manual small incision cataract surgery （MSICS） with intraocular lens implantation （IOL） and trabeculec- tomy （combined triple-operation）on the treatment for glaucoma co- existing with cataract. Methods Combined triple-operation were performed on 57 patients（64 eyes） suffering from glaucoma coexis-ting with cataract. Anterior chamber was pierced through scleral tunnel incision with invert trapezoidal scleral flap, nuclear chop was applied to the operation. The visual acuity, intraocular pressure （IOP） , filtering bleb and complications were investigated after op- eration. Results Mean follow-up was 6.5 mouths （3-18 mouths） , 48 eyes（75% ） had the best corrected visual acuity （BC- VA） at or above 0.3, 6 eyes had worse visual acuity because of op- tic atrophy caused by glaucoma. Normal lOP was found in 58 eyes （91%） , functional filtering bleb in 54 eyes, the lOP was normal without applying any drugs after operation, the mean intraocular pressure（lOP） was reduced to （ 15. 57 ± 3, 41 ） mmHg from （33.90 ± 6.09） mmHg before operation. No serious complications occured in or after operation. Conclusions The combined triple- operation has the following advantages： smaller incision, less tissue damage, lOP of control, more rapid visual recovery and less com- plications. The improvement of visual acuity and lOP control is ac- complished in one operation. Although higher surgical techniques are required, it is an economic, effective, practical method in the treatment for glaucoma coexisting with cataract.
