The relation between shape and size of the continuous curvilinear capsulorhexis with posterior capsule opacification and the position of intraocular lens after cataract surgery

摘要 目的:研究环形撕囊的形状和大小与后发障形成以及术后人工晶体位置之间的关系。方法:对512例(600只眼)行环形撕囊后超声乳化摘除+人工晶体植入术。术后3个月在裂隙灯显微镜下观察患者晶体囊袋结构的变化和人工晶体的位置。结果:A组(410只眼)人工晶体与前囊膜无夹持,B组(126只眼)人工晶体与前囊膜部分夹持,C组(42只眼)人工晶体与前囊膜完全夹持,D组(22只眼)人工晶体与前囊膜虽无夹持但前囊膜环与后囊膜发生紧密粘连。经χ2检验,发现:(1)中心后囊膜发生混浊,C组、D组大于B组(P<0.01),B组大于A组(P<0.01)。(2)人工晶体光学面在眼内的正位率上A组大于C组(P<0.01),C组大于B组和D组(P<0.01)。结论:在排除了患者非手术因素的影响后,白内障超声乳化中环形撕囊位置、形状和大小是术后后囊膜中心混浊的发生率以及人工晶体光学面正位率的关键。 Objective: To evaluate the relation between shape and size of the continuous curvilinear capsulorhexis (CCC) with posterior capsule opacification (POD) and the position of intraocular lens (IOL) after cataract surgery. Methods: Phacoernulsification and CCC implantation were performed in 512 cases (600 eyes). The changes in lens capsule and the position of IOL were observed using slit lamp micrcscope 3 months after operation. Results.Four different kinds of relationship between anterior capsule opening and IOL optic surface were found, including the non- capture (410 eyes) in group A, partial - capture (126 eyes) in group B, total- capture (42 eyes) in group C and non- capture but conglutination between anterior and posterior capsule in group D (22 eyes). (1)The incidence of central POD was significantly higher in group C and D than that in group B and A(P 〈 0.01 ). (2) IOL in well centered position was significantly higher in group A than that in group B and group D (P〈 0.01 ). Conclusions:Obviating non- operation factops, our study suggested that suitable position, shape and size of CCC in cataract transonic emulsionize be key points to avoiding the occurrence of POD and IOL decentration.
