摘要
目的:研究肝移植发生急性排斥反应后,肠内营养(EN)是否干预他克莫司(FK506)对外周血单个核细胞(PBMC)细胞因子的表达。方法:建立SD大鼠原位肝移植模型,随机分为四组,即EN组:给予安素920.5 kJ/(kg·d)、FK组:用FK506 0.8 mg/(kg·d),自然进食、免疫抑制(FN)组:联合应用安素+FK506和对照组:自然进食,均于术后1-7 d进行。术后第8天检测移植受鼠PBMC的白细胞介素(IL-2和IL-10)、γ干扰素(IFN-γ)、转化生长因子-β(TGF-β)等Th1/Th2型细胞因子mRNA表达。结果:肝移植后各细胞因子表达,EN组与对照组无显著性差异(P〉0.05)。FK组和FN组IL-2、IL-10、IFN-γ和TGF-β表达显著降低(P〈0.01),FK组与FN组比IL-10、IFN-γ有显著性差异(P〈0.01)。结论:大鼠肝移植急性排斥反应时,PBMC高表达Th1和Th2型细胞因子。应用FK506在抑制IL-2,减轻排斥反应的同时,其他细胞因子也呈低表达。早期EN可能通过保护肠道黏膜,减轻术后炎性反应,产生与FK506一致的协同效应。
Objective: To investigate the effect of enteral nutrition and FK506 on cytokine expression of PBMC in rats after liver transplantation.Methods: In ROLT models,rats only taking food naturally were randomized to Control group.In 3 experimental groups(EN,FK and FN groups),rats could take food naturally and were either fed with Ensure(920.5 kJ/kg·d),FK506(0.8 mg/kg·d) or both of them separately.7 days later,the expression levels of IL-2,IL-10,IFN-γ,TGF-β mRNA in PBMC were measured. Results: EN group expressed the same level of cytokines as Control group(P〉0.05).FK and FN groups showed significantly lower expression of levels of IL-2、IFN-γ、IL-10、TGF-β compared to Control group(P〈0.01),especially in IFN-γ and IL-10 in FN group(P〈0.01). Conclusion: As acute rejective reaction occurs,host's PBMCs express higher levels of Th1 and Th2 type cytokines.By taken FK506,the cytokines be suppressed significantly.Enteral nutrition could bring into synergistic effect on FK506 by maintaining entero-integrity and ease inflammatory reaction.
