长托宁对常温闭胸式体外循环炎症反应的影响

摘要 目的研究长托宁对常温闭胸式体外循环血清炎性细胞因子浓度的影响。方法20只健康成年家兔随机分为对照组(S组)和长托宁组(C组),每组10只,建立常温闭胸式体外循环模型,于CPB转机前即刻(T1)、转机5分钟(T2)、转机60分钟(T3)、CPB停机后60分钟(T4)四个时点取右颈静脉血,测定血清血栓素B2(TXB2)、内皮素-1(ET-1)及血清肿瘤坏死因子(TNF-α)的浓度。结果转机后各组血清TNF-α、TXB2、ET-1的浓度明显升高,其中TNF-α、ET-1于T2时刻达峰,此后逐渐降低,至T4时刻TNF-α水平仍高于T1时刻,而ET水平降至T1时(p<0.05);TXB2在转机后升高,T3时刻达峰,随后下降,T4时刻仍高于T1时刻水平(p<0.05)。C组在T2、T3、T4时点血清TNF-α、TXB2、ET-1的浓度均低于S组(p<0.05)。结论长托宁能够降低常温闭胸式体外循环时家兔血清TNF-α、ET-1、TXB2水平。 Objective To investigate the effect of penehyclidine hydrochloride on the inflammatory cytokines in normal-temperature car- diopulmonary bypass ( CPB ). Methods 20 rabbits were randomly divided into contrast groups ( group S ) and group penehyelidine hydrochloride (group C) , 10 per group. Normothermia non-chest opening cardiopulmonary bypass (CPB) was performed. And at each of the 4 time points T1 ( the moment just before it began) ,T2 ( 5 minutes after it performed) ,T3 (60 minutes after it performed) and T4 ( 60 minutes after it stopped performing) , blood in jugular vein was sampled to test tumor necrosis factor (TNF - α), thromboxan B2 ( TXB2 ) and level of serocym endothelin - 1 ( ET - 1 ). Results After CPB, the content of TNF - α ,TXB2 and ET - 1 were significantly increased in each group. The content of TNF -ce and ET - 1 reached to the top at time point T2, and decreased afterwards (p 〈 0. 05). Content of TXB2 reached to the top at T3 , and decreased afterwards (p 〈 0.05). Content of TNF - α, TXB2, ET - 1 at T2 ,T3, T4 were higher in group S than that in group C (p 〈0.05 ). Conclusion Penehyclidine hydrochloride applied to cardiopulmonary bypass (CPB) can lower the level of inflammatory cytokines.
