摘要
目的:探讨治疗严重粉碎性髌骨骨折的方法。方法与结果:①5例进行髌骨切除或部分切除术;②10例行单纯钢丝荷包缝合固定;③23例克氏针、钢丝张力带固定,术后髌骨切除或部分切除术及单纯钢丝荷包缝合固定组均需石膏固定,克氏针、钢丝张力带固定组不需石膏固定,d2即可开始行股四头肌等长收缩及直腿抬高锻炼,1周后可进行膝关节持续被动活动(CPM),2周后可进行膝关节不负重主动屈伸活动扶双拐站立行走。结论:克氏针、钢丝张力带治疗严重粉碎性髌骨骨折,髌骨周围软组织刺激症状较少、手术操作简单、内固定稳定、可早期功能锻炼、愈合时间相对缩短,且较少并发症发生、费用低廉,值得基层医院推广。
Abstract： Objective： To discuss the treatment of serious smashing patella fracture. Method and Resuit：①5 examples carryied on patella excision or the partial excision methods;②10 with routine pure steel wires pressure fixedly;③23 examples cleveland needle, steel wire the tension fixed, after the technique patella excision or the partial excision methods and the pure steel wire purse sutures the fixed group to need the gypsum fixedly, the cleveland needle, the steel wire the tension fixed group do not need the gypsum to be fixed, after 2nd days then start the good stock four muscles and so on the long contraction and the straight leg raise exercise, 1 weeks to be possible to carry on the knee joint to continue passively to move （ CPM）, 2 weeks to be possible to carry on the knee joint not to carry a heavy load expand and contract the activity to hold on own initiative double turn and walk. Conclusion： The cleveland needle, the steel wire the tension treatment serious smashing patella fracture, around the patella the soft tissue stimulates the symptom few, the surgery operate simply, in fixed stable, may the early function exercise, heal the time relatively to reduce, also less illness complications have, the expense are inexpensive, are worth the basic unit hospital promotion.
