目的:探讨使用不同透析器膜材料的血透患者血浆IL1和TNFα的水平变化及其与临床的关系。方法:应用胸腺细胞增殖法和L929细胞杀伤法检测使用CUP、PMMA和PS膜透析器患者在血透中血浆IL1和TNFα样活性。结果:CUP和PMMA膜组血透后1h血浆IL1和TNFα水平显著升高,而PS膜组无变化;三种透析膜组在血透后同一时间点的血浆IL1和TNFα水平无显著差异。结论:血浆IL1和TNFα水平的测定在一定程度上能反映透析膜的生物相容性;
OBJECTIVE To study the changes of plasmic levels of interleukin 1(IL 1) and tumor necrosis factor α (TNFα) in patients undergoing hemodialysis and to analysis the influences of different dialysis membranes on it. METHODOLOGY IL 1 activity of plasma was measured by thymocyte proliferative response assay, and cytolytic activity of TNFα was detected by the L929 tumor cell line assay. Fifty three maintenance hemodialysis patients were included, 20 patients dialyzed with cu prophane CUP(GFE 12, Grambro), 16 with polymethylmetacrylate PMMA(B2 1, 2H, Toray) and 17 with polysulfone PS(F6, Fresenius) membrane dialyzer. Blood was sampled at 0, 1, 2, 4 hour during dialysis. RESULTS Plasma levels of IL 1(0.615±0.083) and TNFα(0.594±0.112) increased significantly at 1 hour as compared to those at 0 hour (0.471±0.099, 0.441±0.108) in patients dialyzed with CUP and PMMA dialyzer, while no significant changes were observed in patients with PS dialyzer. No significant differences of plasmic IL 1 and TNFα activity were found at 1, 2 and 4 hour during hemodialysis for each kind of dialyzer. Complaints and symptoms are more frequently seen in patients with CUP and PMMA dialyzer as compared to patients with PS dialyzer, but no statistical significance was obtained. CONCLUSION Observation of plasmic IL 1 and TNFα activity can be used as a marker of membrane biocompatibility and are associated with some of the HD side effects.
