原发性肝癌患者乙型肝炎病毒核心启动因子变异与IL-10、IL-12、TNF-α及IFN-γ关系的探讨 被引量:1

摘要 目的研究原发性肝癌患者乙型肝炎病毒核心基因启动子变异(HBVBCP)与IL-10、IL-12、TNF-α及IFN-γ关系。方法176例HBV慢性感染者[慢性乙型病毒性肝炎轻、中、重度,肝炎肝硬化,慢性重型肝炎和原发性肝癌(HCC)]。采用PCR微板核酸杂交结合ELISA检测显示技术,对患者血清进行检测HBVBCP区核苷酸(nt)1762碱基A→T和1764G→A联合突变;采用双抗体夹心ELISA检测技术,检测患者血清细胞因子(IL-10、IL-12、TNF-α和IFN-γ)水平。结果HBVBCP变异在HCC组的阳性率为70.0%(14/20)显著高于慢性肝病组的阳性率37.8%(59/156)(P=0.008);HCC组的血清TNF-α水平明显高于慢性肝病组(P=0.000),而HCC组的血清IL-10、IL-12和IFN-γ水平与慢性肝病组的比较差异无显著性(P>0.05)。结论HBVBCP变异与原发性肝癌关系密切,且血清TNF-α在原发性肝癌的发生中可能起到一定的作用。 [Objective] To investigate the relationship between hepatitis B virus (HBV) basic core promoter (BCP) mutation and serum IL-10, IL-12, TNF-α as well as IFN-γ, in hepatocellular carcinoma (HCC) patients. [Methotis] Project subject: 176 patients (Chronic hepatitis B with mild, moderate and sever; hver cirrhosis, Chronic fulminant and HCC) with chronic hepatitis B virus infection were studied. Project methods: The A to T mutation at nucleotide 1762 and G to A mutation at nucleotide 1764 were determined by the method of polymerase chain reaction (PCR) microplate hybridization ELISA in these patients. The serum cytokines(IL-10, IL-12, TNF-α and IFN-γ,) of these patients were measured by specific-ELISA. [Results] The positive rate of HCC was significantly higher than that of chronic liver disease (P =0.008). The serum level of TNF-α in HCC was significantly higher than that of chronic liver disease (P =0.000). There was no significant difference between the groups of HCC and that chronic liver disease group in the serum levels of IL-10 and IL-12 as well as IFN-γ, (P 〉0.05). [Conclusion] HCC correlated with HBV BCP mutants. There was a hight positive rate of HBV BCP mutants in the HCC. The serum levels of TNF-α might play a major role on the development of HCC.
