CT、MR对脑弥漫轴索损伤的诊断价值 被引量:3

The value of CT and MRI in the diagnosis of diffuse axonal brain injury

摘要 目的探讨CT、MR对脑弥漫轴索损伤(DAI)的诊断价值及与临床、预后关系。方法回顾性分析经临床确诊的DAI25例,均做了CT、MR检查。结果25例中CT共发现65个病灶,MR共发现263个病灶,二者差异有显著性(P<0.01),MR对脑内非出血性病灶及脑干、胼胝体病灶的敏感性优于CT(P<0.05)。损伤部位与患者临床预后、GCS评分有统计学差异(P<0.05)。结论CT、MR为DAI的临床诊断、预后判断提供了可靠的影像学依据,MR对脑弥漫性轴索损伤的诊断价值明显优于CT。 Objective To explore the value of CT and MRI in the diagnosis of diffuse axonal injury (DAI) along with correlation between clinic and prognosis. Methods 25 cases with clinically-proved DAI were analyzed retrospectively. All the above patients experienced CT and MR examination. Results 65 lesions were recognized on CT, while 263 lesions were found on MR, with statistically significant difference (P〈0.01). MR showed more sensitive than CT in detecting non-hemorrhagic lesions and lesions in bralnstem and corpus callosum (P〈0.05). There were statistically significant difference between lesion location and prognosis and score on GCS (P〈0.05). Conclusion CT and MRI can offer direct and reliable imaging information for clinical diagnosis and estimation about prognosis in DAI. MR is superior to CT in the diagnosis of DAI.
