摘要
目的:探讨多次手术失败后尿道下裂的治疗方法。方法:自2000年起应用植皮法再造尿道25例,取阴囊或腹股沟中厚皮片缝合固定于阴茎腹侧白膜,卷管再造尿道,并行龟头成形术。用带侧孔单腔硅胶导尿管支撑3～6个月。半年后行尿道吻接术。结果:1例患者手术后出现尿道口狭窄,行尿道外口切开术。2例发生尿瘘,其余无并发症发生,阴茎形态佳,尿道口为矢状裂隙。结论:会阴皮肤适合再造尿道,结合带侧孔单腔硅胶导尿管支撑是治疗复杂尿道下裂的较好方法,有推广价值。
Objective To explore a modified method to repair complex hypospadias with skin graft.Methods Total 25 patients with complex hypospadias were included in the study.The scar on the venture of penis was removed,and a free scrotal or groin skin graft was sutured and fixed to corpora cavernosa.Then neourethra was tubularized and supported with a silicone tube,followed by glanuloplasty.The reconstructed urethra was anastomosed half a year later.Results After operation,most patients had good penis contour and was slit-shaped terminal meatus.Yet 1 patient showed urethral stricture of meatus,which was corrected by meatotomy,and 2 patients developed urethral fistula.No other complication was observed.Conclusion The method is of value to the treatment of complex hypospadias.
