摘要
目的:探讨角膜保存时间与术后免疫排斥反应的关系,检测角膜植片的细胞间黏附因子1(ICAM-1)表达及房水、血清中肿瘤坏死因子α(TNF-α)含量与角膜移植排斥反应的关系。方法:先选用Wistar大鼠15只30只眼随机分3组(每组10只眼),分别用不同方法保存,即:湿房保存12h,中期保存液保存7d,深低温长期保存12周。再选用SD大鼠30只30只眼(右眼)作为受体,分别使用上述不同保存方法的Wistar大鼠眼作为供体建立角膜移植排斥反应动物模型。将SD大鼠随机分3组每组10只眼:Ⅰ组(异体湿房保存组);Ⅱ组(异体中期保存组);Ⅲ组(异体长期保存组)。观察各组角膜移植术后排斥反应指数(RI)、植片存活时间(MST)和植片的病理变化。HE染色及免疫组化染色观察免疫排斥反应的程度及ICAM-1表达程度。采用酶联免疫吸附试验检测各组大鼠角膜移植术后10d时房水、血清中TNF-α含量变化。结果:Ⅲ组RI明显降低,MST明显延长,与其他两组比较均差异十分显著(P<0.01)。角膜移植术后10d时HE染色显示Ⅲ组炎性反应较Ⅰ组、Ⅱ组明显减轻,免疫组化染色显示Ⅲ组ICAM-1表达较Ⅰ组、Ⅱ组明显减少。Ⅲ组眶血清及房水中TNF-α含量明显低于Ⅱ组和Ⅰ组,差异十分显著(P<0.01)。结论:深低温保存的大鼠角膜移植术后免疫排斥反应发生率降低,发生时间延迟。ICAM-1和TNF-α是角膜移植免疫排斥反应发生的重要因素之一。
Objective:To explore the relationship between corneal storage methods and rejection after corneal transplantation, and to study the roles of intercellular adhesion molecule-1 (ICAM-1) and tumor necrosis factor-α (TNF-α) in aqueous humor and serum in keratoplasty rejection in rats. Methods:A total of 15 Wistar rats and 30 SD rats were selected as corneal donor and recipient,respectively. Thirty eyes from 15 Wistar rats were randomly assigned to be preserved with 3 different methods (n=10 each group):moist chamber storage for 12 hours,intermediate-term storage for 7 days,and long-term storage for 12 weeks. Thirty right eyes of 30 SD rats were randomly divided into 3 groups (n=10 each group):group Ⅰ (receiving the Wistar rat cornea treated with moist chamber storage),group Ⅱ (receiving the Wistar rat cornea treated with intermediate-term storage), and group Ⅲ (receiving the Wistar rat cornea treated with long-term storage). The rejection index (RI),means survival time (MST) of corneal grafts,and histopathological changes in corneal grafts were recorded. HE and immunohistochemical staining were performed to observe the intensity of corneal rejection and the level of ICAM-1. The levels of TNF-α in the aqueous humor and serum were determined by enzyme linked immunoserbent assay after 10 days of corneal transplantation. Results:RI significantly decreased and MST was significant longer in group Ⅲ than in other 2 groups (P 〈 0.01 ). The severity of corneal inflammation was less severe and the level of ICAM-1 significantly lower in group Ⅲ than in other 2 groups. Compared with groups Ⅰ and Ⅱ ,the levels of TNF-α in serum and aqueous humor were significantly lower in group Ⅲ (P〈 0.01). Conclusion:The rejection rate after the corneal transplantation of long-term preserved cornea is lower, and the occurrence of the rejection is delayed. Both ICAM- 1 and TNF-α are correlated with the corneal rejection.
