摘要
目的：探讨胃癌淋巴结转移的规律及其意义。方法：回顾性分析我院2005年7月至2006年4月胃癌根治术标本52例共计淋巴结1176枚,平均每例22.6枚（9-60枚）,将淋巴结术中评定为阳性、可疑、阴性3类,阳性淋巴结常规病理检查,可疑、阴性淋巴结切成两半分别保存,可疑且常规病理为阴性的462枚淋巴结行逆转录-聚合酶链反应（RT-PCR）检查,研究不同部位各组淋巴结的转移规律,采用Logistic回归分析淋巴结转移相关的因素。结果：52例胃周淋巴结转移大量存在,有48.3%（223/462）阴性淋巴结存在微转移,肿瘤部位不同（胃窦、胃体部、胃上部）淋巴结转移的顺序和规律不同,跳跃性转移常见。所有淋巴结中No6组淋巴结转移率最高平均为75.7%,其次为No3组为72.9%。No7、8、9组淋巴结转移率分别为65.3%、50.0%和35.9%。与No8、9组淋巴结转移密切相关的因素有肿瘤部位、组织类型、浸润程度（P〈0.05）。结论：肿瘤部位、组织类型、浸润程度不同的胃癌淋巴结转移规律不同,合理选择手术方式进行个体化治疗尤为重要。
Objective： To investigate the lymph node metastases in gastri%carcinoma and its clinical significance. Methods. A total of 1 176 lymph nodes were taken from 52 patients with average of 22.6 nodes per case had been assessed by clinicopathological hematoxylin-eosin （H＆E） staining and reverse transcriptasepolymerase chain reaction （RT-PCR）. Hematoxylin-eosin （H＆E） staining expression was detected in 310 lymph nodes. 462 lymph nodes had been assessed by RT-PCR and 223 lymph nodes micrometastasis. The total metastatic rate was calculated. Binary logistic regression analysis was used to analyze the influence index of five clinicopathological factors on the No 8, 9 and other lymph node metastases which 25-50% metastatic rate had been assessed. Results： The metastases rate （75.7%） of No 6 group lymph node was the highest and seconded by No 3 group （72.9%） . The overall lymph node metastases （LNM） of 52 patients with gastric carcinoma was 48.3 %. The depth of tumor invasion and tumor histiopathologic-classification and tumor location were main factors to influence the LNM rate （P d0.05）. Conclusions： The status of the depth of tumor invasion and tumor histiopathologic-classification and tumor location acts as independent indicating the necessity of further lymph node dissection and individualized treatment.
