摘要
目的总结特殊类型阑尾炎行腹腔镜阑尾切除术（laparoscopic appendectomy,LA）的方法与疗效。方法8例老年、10例儿童和6例异位阑尾炎行LA治疗。结果平均手术时间45（30～90）min,无中转开腹者,术后第1 d即可进食,平均26（16～48）h恢复胃肠功能。无伤口感染、肺部感染等并发症。结论特殊类型阑尾炎行LA具有视野好、伤口感染少等优点,操作过程应根据患者具体情况灵活选用戳孔位置等手术技巧。
Objective To summarize the skills and efficacy of laparoscopic appendectomy（LA） for special appendicitis.Method 24 cases of special appendicitis were underwent LA,including 8 of aged,10 of children,and 6 of ectopic ones.Results The mean operation time was 45（30～90） minutes.No one was conversed to open surgery.All the patients recovered semifluid diet after operation,and the time of gastrointestinal function recovery was 26 hours averagely.No incision or pulmonary infection occurred.Conclusion LA is recommendable for special appendicitis with advantages as good surgical field and less wound infection.It is key point to select port site neatly during the LA process.
