摘要
目的:探讨血清谷氨酸脱氢酶(GLDH)在肝细胞损伤中的临床价值,并对其预后进行判断。方法:在日立7060型生化自动分析仪上,采用酶法对110例正常对照组和126例急慢性肝细胞损伤组血清谷氨酸脱氢酶进行检测,同时监测AST、ALT、T-BIL及γ-GT等常规生化项目,并作相关性分析,统计有关指标。结果:对于肝细胞线粒体损伤严重的疾病,血清中谷氨酸脱氢酶的活性明显高于健康者(P<0.05),其阳性率极高,明显优于其它常用的ALT、AST、T-BIL及γ-GT等酶;其它肝、胆系疾病患者谷氨酸脱氢酶的活性也随病情、病程的变化而有不同程度的升高,其升降与大多数疾病转归相关。结论:谷氨酸脱氢酶为肝细胞线粒体特异性酶,检测血清谷氨酸脱氢酶的活性对肝脏疾病及相关疾病的早期诊断、掌握病情、观察疗效、判断预后等有着独特的临床应用价值,比谷丙转氨酶、谷草转氨酶、γ-谷氨酰转肽酶等更能特异性的反映肝细胞损伤程度,可作为现有肝病临床诊断的协同指标。
Objective :To discuss the clinical value of serum GLDH in patients with liver injury and to estimate the prognosis. Methods:GLDH in 110 healthy controls and 126 patients with liver injury was measured, and AST, ALT, T- BIL and γ- GT were examined with the HITACHI- 7060S clinical chemistry analyzer by enzyme reaction method. Results:The GLDH in the patients with serious injury in the liver cell mitochondrion was significantly higher than that of the control group (P〈0.05) . The GLDH was superior to other enzymes, such as AST,ALT and γ-GT in the positive rate. The GLDH in other patients with hepatobiliary system diseases increased with the changes in the condition and course of disease. Its rise and fall was related to prognosis of disease. Conclusion:GLDH is a specific enzyme to mitochondrion of liver cells. Detecting GLDH activity is of significance in the early clinical diagnosis, curative effect observation and prognosis estimation of liver diseases and its relative diseases. GLDH is more specific than AST,ALT,T-BIL and γ-GT in reflecting the degree of liver injury. It can be used as a coordinative indicator to diagnose liver diseases.
