摘要
通过鼻咽原发癌与颈部淋巴结转移灶的对比观察,提出了转移癌的形态特点有:1、癌型多向泡状核细胞癌转化;2、癌细胞呈片块型、条索型及硬化型分布,以片块型分布多见,以条索型及硬化型为其相对特点;3、癌细胞合体状、大小形态相对一致,没有显著的异型性,核仁粗大、中位、嗜酸;4、癌巢内有较明显的淋巴细胞浸润,网状纤维多为间断性包裹癌巢。文中对颈部淋巴结转移性鼻咽癌的病理诊断标准及鉴别诊断提出了意见,对鼻咽癌颈部淋巴结转移的机制进行了探讨。
Comparing the Primary foci of nasopharyngeal carcinoma with the cervical lymph nodal metastases,the pathological morphological features in the latter are as follows:1,The carcinoma type transforms mostly to the vesicular form;2,The distribution of the cancer cells appears in the form of sheets,cordlike strands and sclerosing.Although most of them are in the form of sheets,the cordlike strands and sclerosing are relatively more characteristic;3,The cancer cells have ill-defined boundaries,their sizes and shapes are rather similar,and there is no striking atypia.The neucleoli are prominent,central and eosinophilic;4,There is significant lymphocytic infiltration into the cancerous nests which are surrounded mostly by disconti- nuous reticular fibers. The criteria for diagnosis and differential diagnosis are presented and the mechanisms of the metastasis of nasopharyngeal carcinoma to the cervical lymph nodes are discussed.
