摘要
目的探讨前路减压手术治疗颈脊髓损伤的治疗效果。方法应用颈前路手术治疗颈椎骨折脊髓损伤24例。本组中,骨折者16例,脱位者6例,骨折并脱位者2例。损伤部位:C4:4例,C5:8例,C6:8例,C7:4例。发生四肢不全瘫痪者20例,四肢完全瘫者4例。根据北医三院的评定标准,肢体残疾一级者6例,二级者10例,三级者8例。平均伤史6.5d(3h^8周)。结果术后随访0.5~6年,平均2.5年,根据北医三院的疗效标准,结果为:优5例(20.8%),良7例(29.2%),有效4例(16.7%),差8例(33.3%)。优良率为50.0%,总有效率为66.7%。结论前路减压手术治疗颈脊髓损伤可获一定疗效。
Objective To explore the surgical effect of treating the cervical spinal cord injury by the anterior decompression. Methods Total 24 patients suffered from the cervical spinal cord injury with fracture and dislocation underwent the anterior decompression. There were 16 cases with cervical fracture, 6 cases with dislocation, and 2 cases with cervical fracture and dislocation in this group. The injury level was C4 in 4 cases, Cs in 8, C6 in 8, C7 in 4. According to the classification of 3rd Hospital of Beijing Medical University, grade Ⅰ was in 6 cases, grade Ⅱin 10, grade Ⅲin 8. The mean of the injury history was 6. 5 days with a range of 3 hours -8 weeks. Results All patients obtained follow - up for 0. 5 -6 years with a mean of 2. 5 years. The results were the excellent in 5 cases （20. 8% ） , good in 7 cases （29. 2% ）, fair in 4 cases （16. 7% ）, and poor in 8 cases （33. 3% ）. The rate of the excellent and good was 50%. The total effective rate was 66. 7%. Conclusion The better effect of treating the cervical spinal cord injury can be obtained by the anterior decompressive surgery.
