覆膜支架在周围血管病变中的应用 被引量:22

摘要 目的回顾性分析覆膜支架治疗周围血管性病变的方法和疗效。方法11例患者,其中动脉瘤8例(包括假性动脉瘤3例),动-静脉瘘2例,血管损伤破裂1例。病变部位位于锁骨下动脉2例、颈总动脉1例、肾动脉1例、髂动脉5例、股动脉1例、胫前动脉1例。均采用经皮股动脉穿刺,在相应的病变段植入覆膜支架,使病变与主支血管隔绝。结果所有患者均成功植入覆膜支架,术后造影原病变完全消失。术中有1例发生血管痉挛。未发生血管闭塞、破裂等并发症。通过超声或DSA随访3~6个月,无复发和主支血管闭塞等。结论随着介入技术的不断发展,覆膜支架的应用日益增多,治疗周围血管性病变取得极好效果。 Objective To evaluate the clinical application of covered stent in peripheral vascular disease by retrospective analysis of 11 patients. Methods 11 patients(male 5, female 6, mean age 45y)were undertaken angiography and diagnosed as aneurysm in 8 patients, arteriovenous fistula in 2, and vascular injury in 1; with 2 involving subclavian artery, 1 common carotid artery, 1 renal artery, 5 iliac artery, 1 femoral artery, and 1 anterior tibial artery. All the patients underwent covered stent implantation to isolate the diseased segment from the vascular trunk through percutaneous femoral artery approch. Results All the patients were successfully implanted with covered stentt, and the lesions vanished on angiography after operation. During the procedure, 1 patients showed vasospasm but no other complications such as thrombosis or angiorrhexis. No recurrence and no vascular occlusion were found after follow-up of 3 - 6 months by B ultrasonic or DSA. Conclusions Covered stent implantation is safe, efficient for treatment of peripheral vascular disease and worthy to be recommended. (J Intervent Radiol, 2007, 16: 594-597)
