摘要
目的：认识正常小肠血管形态及小肠平滑肌肿瘤血管形态。方法：对８例正常成人新鲜小肠标本、１０例（１５个瘤体）经病理证实的手术切除标本，分别采用血管铸型、血管造影、解剖镜观测及图像分析。结果：空肠段直动脉通常分三支——两长支、一短支；回肠段直动脉通常分两支——长、短各一支；空－回肠段呈移行改变，直动脉在进入肠壁之前可见吻合。血管密度以十二指肠球部最低，在空回肠呈现由低增高，继而降低的变化规律。平滑肌肿瘤血管增粗、丛状增生、吻合丰富，形似“蜘蛛网”状，出现造影剂池、血管纡曲、中断征，提示平滑肌肉瘤。与正常组比，平滑肌肿瘤血管面密度（目标总面积／统计场总面积）显著性增多（Ｐ＜０．０５）。结论：认识正常小肠血管分布规律，有助于小肠肿瘤的介入诊断和治疗。小肠平滑肌肿瘤为多血供肿瘤，良、恶性之间存在一定的血管特点。
Objective: To establish the normal vascular anatomy compare it with that of leiomyomatous tissue in the human small intestine. Methods: Eight biopsy specimens from normal adults and 10 from 15 pathologically confirmed leiomyomas from 10 patients were studied with vascular cast, angiography and quantitative analysis. Results: The vasa recta of the jejunum was composed of two long and one short branches while that of the ileum were composed of one long and one short branches. There were anastomoses between vasa recta before they entered the intestinal wall. The vascular density was the lowest in the duodenal bulb, increased from low to high in the jejunum and then went down in the ileum. The vasculature of leiomyomatous tissue looked like a spider net with large lumen, proliferation in bundles, rich anastomosis, high vascular tortuosity and interruption sign. There was pooling of contrast medium in leiomyosarcoma. The vascular density was significantly higher in the leiomyomatous tissue than in the normal one (P<0.05). Conclusion: Understanding of the vascular distribution in the normal intestine is helpful for the interventional diagnosis and therapy of intestinal tumors. The leiomyomatous neoplasms in the small intestine are rich in vascularity and the benign and malignant tumors possess different vascular features.
