摘要
目的探讨福建省妊娠期梅毒发病率及其对围产儿的影响。方法对2004年7月1日至2006年6月30日纳入调查对象,均在知情同意下进行快速血浆反应素(RPR)筛查,凡RPR阳性者进一步检测RPR滴度,同时行梅毒螺旋体血凝试验(TPHA)以确诊。对确诊为妊娠期梅毒孕妇进行跟踪调查直至分娩。并对每个确诊为妊娠期梅毒的孕妇进行产前问卷调查,对其所生新生儿填一份随访表。结果共有调查对象49575例,其中接受RPR筛查孕产妇45529例,查出RPR阳性并经TPHA确诊的妊娠期梅毒847例,其中已分娩772例,妊娠期梅毒发病率为1.70%(772/45529);绝大多数为潜伏梅毒;其围产儿死亡率为59.74‰、低体重发生率8.05%、早产率7.40%,均明显高于同期分娩非妊娠期梅毒之围产儿以上各率,P<0.01差异有统计学意义;围产儿预后与孕母RPR滴度有关,孕母RPR滴度<1∶8与滴度≥1∶8的比较,滴度高对围产儿的影响大,预后差,围产儿死亡率、早产率、低出生体重发生率高,P值均<0.01,差异有统计学意义。结论福建省妊娠期梅毒发病率逐年升高,绝大多数为潜伏梅毒;其围产儿死亡率、早产率、出生低体重发生率均高于非妊娠期梅毒之围产儿的以上各率;围产儿预后与孕母RPR滴度有关。
Objective To explore the prevalence and impact of Syphilis during pregnancy on neonates in Fujian province.Methods Agreeing in advance, cases in pregnancy were tested by the Rapid Plasma Reagin test （RPR） and confirmed by Treponnna Pallidum test （TP） from July 1th, 2004 to June 30, 2006.The cases diagnosed answered a questionnaire before delivery and were followed up until delivery, the neonates were followed up as well. Results Total 49 575 cases were investigated. 45 529 were tested by RPR, in which 847 cases were tested and confirmed to be possitive.There were 772 cases had deliveried.The prevalence of syphilis during pregnancy was 1.70% （772/45 529） . Most cases were latent syphilis. The neonatal mortality was 59.74‰, the low birth weight rate was 8.05‰ and the preterm birth rate was 7.40 ‰, all higher than that of the non-syphilis in the same period （ P 〈 0.01）. The higher mother blood RPR titer was, the worse prenatal prognosis was with the higher neonatal moratlity, preterm birth rate and low birth weight rate （P〈0.01）. Conclusion The prevalence of syphilis during pregnancy is going up yearly, underlying syphilis mostly.The neonatal moratlity, preterm birth weight rate and low birth rate are higher than that of the non-syphilis in the same period. The prenatal prognosis is related to the mother＇s blood RPR titer.
