肾移植术后监测CsA血药浓度C_2和C_0对指导CsA合理应用的临床价值 被引量:4

Clinical value of supervising rational application of CsA by monitoring CsA concentration C2 and C0 in kidney transplant recipients

摘要 目的:比较监测肾移植术后服用环孢素A(CsA)的血药浓度峰值(C2)和谷值浓度(C0)对判定CsA抗排斥疗效及毒副作用的临床价值。方法:采用免疫荧光偏振TDX法监测365例肾移植受者CsA浓度,其中212例监测CsA峰值浓度(C2组),153例监测CsA谷值浓度(C0组)。分析两组CsA浓度测定值与患者移植肾功能、急性排斥反应、药物性肝肾中毒以及肺感染相关临床资料的关系。结果:急性排斥反应的发生率C2组(19.8%)明显低于C0组(26.1%),两组比较差异有显著性(P<0.05);药物性肝、肾中毒及肺感染的发生率C2组分别为18.3%、10.5%和13.7%,C0组分别为20.3%、9.9%和14.2%,两组比较差异均无显著性(P>0.05)。结论:CsA血药浓度C2和C0监测都是肾移植术后药物调控的有效指标,C2在监控急性排斥方面意义更大。
