摘要
目的探讨经尿道远端输尿管拔出术在治疗肾盂输尿管癌中的应用价值。方法肾切除后,从远端输尿管放置输尿管导管入膀胱,然后经尿道置入电切镜,以针式电极将输尿管口周围膀胱壁环形电切至肌层,再经尿道将输尿管远端套叠拔出。已采用此法治疗4例肾盂癌。结果4例患者术后无明显出血,无感染、尿瘘等并发症,经平均16个月随访,未发现环切口肿瘤种植,亦未发现膀胱和其他部位发生肿瘤。结论该术式操作简单,易于掌握,手术创伤小,手术时间缩短,并发症少,效果好,是治疗肾盂及上段输尿管肿瘤的有效手术方法之一。
Objective To study the application of the transurethral extracting resection of lower ureter in the radical operation of pelvis or ureter cancer. Methods After resection of the kidney, a catheter was put into bladder through the lower residual ureter. The ring-shape wall of bladder involving the almost muscle layer around the orifice of ureter was resected using the pin-shape electrode under the resectoscope through urethra and then the lower residual ureter was extracted out of the body through the sheath of the resectoscope. Four patients with pelvis or ureter cancer were treated in this manner. Results For these 4 patients,there was no complication of bleeding, infection or urinary fistula. A average 16-month follow-up was perfnreled. No tumor plantation or recurrence in the bladder or anywhere else in the body was found. Conclusions This operation is a effective way for pelvis or ureter cancer because of its characteristics, such as the simpleness, easy to master,ligher wounded, short operation time,less complications and the ideal effects.
