全胃切除术后早期肠内营养对免疫状态的影响 被引量:5

摘要 目的研究全胃切除患者术后早期肠内营养支持对机体免疫功能及炎症反应的影响。方法选择2004年3月~2006年11月因胃癌行根治性全胃切除患者76例,随机分为两组:PN组(对照组)38例术后接受外周静脉营养;EN组(实验组)38例,术后24小时开始肠内营养治疗(连续7天)。基本等热量,并对两组患者于术前及术后第1日、第8日分别抽取外周血测定血清IgG,IgA,IgM,T淋巴细胞亚群,IL2及TNFα。结果术后第8日实验组的IgG,IgM,IgA,CD4和CD8均明显高于PN组(P<0.05),而TNFα明显低于PN组(P<0.05)。结论胃癌根治性全胃切除患者术后早期肠内营养支持可有效改善患者的免疫功能,并减轻机体的炎症反应。
