大量失血患者合理使用血浆研究 被引量:2

STUDY ON THE INFLUENCE OF REASONABLE TRANSFUSION FFP IN PATIENTS WITH MASSIVE HEMORRHAGE

摘要 [目的]研究根据现代新鲜冰冻血浆(FFP)输注适应症标准合理使用血浆对大量失血患者产生的影响。[方法]选择我院2005-2006年失血量达30%以上的大量失血患者,根据是否采用现代FFP输注适应症标准分为非控制组(79例)和控制组(86例),回顾性分析两组患者的输血情况及相关指标。[结果]非控制组人均用血量(24.4±10.4)u,其中64例输注FFP,人均FFP用量(6.4±4.5)u,人均RBC用量(23.1±11.4)u;控制组人均用血量(21.5±8.4)u,其中22例输注FFP,人均FFP用量(11.8±3.2)u,人均RBC用量(17.8±8.2)u;两组比较差异有统计学意义(P﹤0.05)。两组病人(排除死亡病例)出院前Hb、Plt、PT、APTT、Alb水平均恢复正常;非控制组输血不良反应发生率为10.1%,感染率36.7%;控制组输血不良反应发生率为2.3%,感染率24.4%;两组比较差异有统计学意义(P﹤0.05)。非控制组死亡率7.6%;控制组死亡率7.0%,两组比较差异无统计学意义(P﹥0.05)。[结论]根据现代FFP输注适应症标准合理使用FFP,不仅直接减少FFP用量,而且由于有效纠正了凝血因子缺乏所致的出血,从而减少了红细胞制品用量,并显著降低大量失血患者输血不良反应发生率和感染率。 [ Objective ] To research the influence of reasonable transfusion FFP in patients with massive hemorrhage based on file current transfusion indication about FFP. [Metheds] The patients with massive hemorrhage were studied. They were divided into transfusion group (79 cases) and control group (86 cases) by whether they were qualified with the current transfusion indication about FFP. The situation of transfusion and correlated indices of the two groups were retrospectively analysed. [Results] The average volume of blood transfused was (24.4 ± 10.4) u in transfusion group (79 cases), among which 64 cases were infused FFP, and the average volume of FFP and RBC was (6.4 ± 4.5) u and (23.1 s- 11.4) u of the 64 cases. The average volume of blood transfused was (21.5 ±8.4) u in control group (86 cases), among which 22 cases were infused FFP, and the average volume of FFP and RBC was (11.8 ± 3.2) u and (17.8 ±8.2) u of the 22 cases. There were significant differences between the two groups (P〈 0.05). The levels of Hb, Pit, PT, APTT and Alb were all anastated to nornmal rang before leaving hospital in the two groups (death cases were depleted). The rates of transfusion reaction and infectian were 10.1% and 36.7% in transfusion group, and those were 2.3% and 24.4% in control group. There were significant differences between the two groups (P〈 0.05). The death rate (7.6% in non-control group, 7.0% in control group) was not significant (P〉 0.05). [Conclusion] Reasonable transfusion FFP in the patients with massive hemorrhage based on the current improvement transfusion indication about FFP can effectively reduce the volume of FFP, at the same time reduce the vohnne of RBC and significantly reduce the rates of transfusion reaction and infection, because of improving the blood coagulation function and correcting the bleeding.
