摘要
[目的]了解常州市钟楼区5岁以下儿童死亡率及影响因素,为政府部门制定妇幼保健决策提供依据。[方法]采用流行病学前瞻性调查方法,以妇幼三级保健网为依托,对2002-2005年常州市钟楼区出生的6 665例活产儿及5岁以下儿童死亡情况进行全面监测。[结果]4年间婴儿死亡率为5.40‰,5岁以下儿童死亡率为7.20‰,新生儿及婴儿死亡占5岁以下儿童死亡的比例分别为47.92%、75.00%。婴儿及5岁以下儿童死因顺位均为新生儿疾病、先天畸形发展、意外。[结论]常州市钟楼区3个死亡率虽然低于全省水平,但有波动,提示我们应重点加强先天畸形、早产及低出生体重、出生窒息的保健管理。意外伤害、肿瘤导致5岁以下儿童死亡呈上升趋势,应引起足够重视。提出应加强婚前检查,开展出生缺陷的筛查,建立新生儿急重症急救网络,强化健康教育,提高妇幼保健质量和产儿科适宜水平等措施,以便进一步降低死亡率。
[Objective] To analyze the mortality rate of children under 5 years old and its influential factors in Zhonglou district of Changzhou, and provide basis for government to make strategies for the health care of women and children. [ Mothods] By prospective study of epidemiology, based on the three-level network of women and children health care, an overall monitoring and analysis of 6 665 cases of live births and death situation of children under 5 years old during 2002-2005 were obtained. [ Results] During the four years, infant mortality rate was 5.40‰, mortality rate of children under 5 years old was 7.20‰, the proportions of neonatal and infant death to deaths of children uader 5 years old were 47.92% and 75.00% respeclively. The death causes of infant and children trader 5 years old were neonatal diseases, congenital malformation and accident in sequence. [ Conclusion] The three mortality rates in Zhonglou district of Changzhou are lower than the average level of Jiangsu province, but there are undulation among them, which indicates that we should focus on health care of congenital malformation, premature and low weight neonate as well as neonatal asphyxiation. We should pay enough attention to the increasing death of children under 5 years old caused by accident and tumor. To further reduce the mortality rates, we should improve premarriage examination, develop screening of birth defect, establish the emergency network of neonate, strengthen health education, improve the quality of women and children health care and appropriate technologic level of obstetrics and pediatrics,
