摘要
[目的]探索以诊疗为主的外展服务对女性性工作者的干预效果。[方法]对女性性工作者进行外展服务干预(性病艾滋病预防知识宣传、安全套的免费发放和免费诊疗等),通过问卷调查和检验,比较干预前后研究对象对性传播疾病艾滋病知晓率、安全套使用率、求医行为和性传播疾病感染率的变化。[结果]研究对象对性病艾滋病和安全套使用知识知晓率显著提高(P﹤0.05);求医行为差异有统计学意义(χ2=21.3912,P﹤0.00),个体诊所就诊率从29.6%下降到9.7%,正规医院就诊率从59.2%提高到78.2%;性传播疾病感染率从74.3%下降到60.8%(χ2=7.405,P﹤0.00);霉菌性阴道炎从12.1%下降4.6%(χ2=6.181,P﹤0.01),细菌性阴道病从44.2%下降到18.3%(χ2=26.523,P﹤0.00)。[结论]外展服务促进女性性服务工作者性病艾滋病知识的掌握、求医行为的改变,降低部分可治愈性性传播疾病感染率。
[Objective] To study the effect of the intervention of outreach service on female sex workers. [Methods] 206 female sex workers before intervention were included. The intervention program included propagandizing the related knowledge about sex transmitted diseases （STDs） and AIDS, the free distribution of STDs/AIDS prevention leaflets and condom, free diagnosis and treatment of STDs by outreaching mode. The rate of the awareness of the related knowledge about STDs/AIDS, the use of condom, the infective situation of STDs of female sex workers and the behavioral change of visiting the doctors between pre-intervention and post-intervention were compared. [ Results] The awareness rate of the knowledge about STDs/AIDS and the rate of condom use increased significantly （P〈 0.05） . The behavior of visiting doctors was changed （X^2 = 21.3912, P 〈 0.00） ： the percentage of visiting the privacy clinics reduced from 29.6% to 9.7% and the percentage of visiting the normal hospitals increased from 59.2% to 78.2%. The infection rate of the curable STDs declined from 74.3% to 60.8% （ X^2 = 7.405, P 〈 0.00）, the infection rates of mildew vagina and bacilli vagina diseases reduced from 12.1% to 4.6% （P 〈 0.01 ） and from 44.2% to 18.3% （P 〈 0.00） respectively. [Conclusion] The comprehensive intervention of outreach service is an effective wayto improve the awareness of STDs/AIDS, the behavior of visiting doctors and to reduce the infection rate of some curable STDs.
